<p>The research presented in this article determined the systemic (blood plasma) and local (nasal lavage fluid) levels of interleukin-17 (<abbrev xlink:title="interleukin-17" id="ABBRID0E6E">IL-17</abbrev>) and interleukin-33 (<abbrev xlink:title="interleukin-33" id="ABBRID0EDF">IL-33</abbrev>) in healthy volunteers, patients with allergic rhinitis (<abbrev xlink:title="allergic rhinitis" id="ABBRID0EHF">AR</abbrev>), and patients with both allergic rhinitis and recently diagnosed bronchial asthma (<abbrev xlink:title="allergic rhinitis" id="ABBRID0ELF">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EPF">BA</abbrev>). <abbrev xlink:title="interleukin-17" id="ABBRID0ETF">IL-17</abbrev> and <abbrev xlink:title="interleukin-33" id="ABBRID0EXF">IL-33</abbrev> levels were consistent between <abbrev xlink:title="allergic rhinitis" id="ABBRID0E2F">AR</abbrev> and <abbrev xlink:title="allergic rhinitis" id="ABBRID0E6F">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EDG">BA</abbrev> patients but diverged when comparing either patient group or the combined group of all patients with healthy volunteers. <p>Allergic rhinitis (<abbrev xlink:title="allergic rhinitis" id="ABBRID0EXBAC">AR</abbrev>) and bronchial asthma (<abbrev xlink:title="bronchial asthma" id="ABBRID0E2BAC">BA</abbrev>) are inflammatory diseases of the upper and lower respiratory systems, respectively, and represent a significant burden on healthcare systems worldwide (<xref ref-type="bibr" rid="B6">Cardell et al. 2016</xref>; <xref ref-type="bibr" rid="B46">Nurmagambetov et al. 2018</xref>; <xref ref-type="bibr" rid="B4">Avdeeva et al. 2020</xref>; <xref ref-type="bibr" rid="B51">Roland et al. 2021</xref>; <xref ref-type="bibr" rid="B22">Håkansson et al. 2023</xref>). Estimates of the prevalence of <abbrev xlink:title="allergic rhinitis" id="ABBRID0ETCAC">AR</abbrev> vary from 5 to 50%, depending on the methodology and population studied (<xref ref-type="bibr" rid="B48">Oliveira et al. 2020</xref>; <xref ref-type="bibr" rid="B2">Alqahtani 2020</xref>). Epidemiological studies with a more restrictive definition of <abbrev xlink:title="allergic rhinitis" id="ABBRID0E6CAC">AR</abbrev>, incorporating IgE plasma levels to confirm the presence of atopic immune responses, report prevalences of 10–12% (<xref ref-type="bibr" rid="B62">Tschopp et al. 1998</xref>; <xref ref-type="bibr" rid="B53">Salo et al. 2011</xref>). The reported prevalence of <abbrev xlink:title="bronchial asthma" id="ABBRID0ELDAC">BA</abbrev> ranges from 5.4 to 17.9% (<xref ref-type="bibr" rid="B56">Song et al. 2022</xref>).</p>
<p>Available data indicate that the pathogeneses of <abbrev xlink:title="allergic rhinitis" id="ABBRID0EVDAC">AR</abbrev> and <abbrev xlink:title="bronchial asthma" id="ABBRID0EZDAC">BA</abbrev> are linked. In 1997, J. Grossman published the “one airway, one disease” theory, which states that asthma and allergic rhinitis are “described better as a continuum of inflammation involving one common airway” than as distinct and separate entities (<xref ref-type="bibr" rid="B21">Grossman 1997</xref>). Data from the SAPALDIA cohort study published the following year show that 60% of patients with <abbrev xlink:title="bronchial asthma" id="ABBRID0EBEAC">BA</abbrev> also suffer from <abbrev xlink:title="allergic rhinitis" id="ABBRID0EFEAC">AR</abbrev> (<xref ref-type="bibr" rid="B62">Tschopp et al. 1998</xref>). In 1999, data from the ECRHS showed that patients with <abbrev xlink:title="bronchial asthma" id="ABBRID0ENEAC">BA</abbrev> are likely to be co-diagnosed with <abbrev xlink:title="allergic rhinitis" id="ABBRID0EREAC">AR</abbrev> (<xref ref-type="bibr" rid="B34">Leynaert et al. 1999</xref>), and updated data from the same study confirmed <abbrev xlink:title="allergic rhinitis" id="ABBRID0EZEAC">AR</abbrev> as a risk factor for <abbrev xlink:title="bronchial asthma" id="ABBRID0E4EAC">BA</abbrev> (<xref ref-type="bibr" rid="B55">Shaaban et al. 2008</xref>). More recent studies show a similar connection (<xref ref-type="bibr" rid="B28">Jacob et al. 2016</xref>; <xref ref-type="bibr" rid="B1">Acevedo-Prado et al. 2022</xref>).</p>
<p>Additional evidence for the one airway theory of <abbrev xlink:title="allergic rhinitis" id="ABBRID0EPFAC">AR</abbrev> and <abbrev xlink:title="bronchial asthma" id="ABBRID0ETFAC">BA</abbrev> lies in the morphology and cytology of the respiratory epithelium, a key tissue in the inflammatory processes of the upper and lower respiratory tracts. The major component of the respiratory epithelium throughout the airways is the ciliated columnar epithelial cells, which make up 50–80% of the tissue by cell count. Additional major cell types include goblet cells, which produce mucus (<xref ref-type="bibr" rid="B30">Jaramillo et al. 2018</xref>); basal cells, which are the major population of stem cells in this tissue (<xref ref-type="bibr" rid="B12">Cumplido-Laso et al. 2023</xref>); and chemosensory cells, which serve as initiators of physiological responses to external stimuli by interfacing with the immune and nervous systems (<xref ref-type="bibr" rid="B26">Hollenhorst and Krasteva-Christ 2023</xref>).</p>
<p>Evidence for the interplay between <abbrev xlink:title="allergic rhinitis" id="ABBRID0EFGAC">AR</abbrev> and <abbrev xlink:title="bronchial asthma" id="ABBRID0EJGAC">BA</abbrev> has also been found in the clinical setting. A 3-year, randomized, open-label trial of immunotherapy in 205 children with <abbrev xlink:title="allergic rhinitis" id="ABBRID0ENGAC">AR</abbrev> showed that those receiving immunotherapy were less likely than the control group to develop <abbrev xlink:title="bronchial asthma" id="ABBRID0ERGAC">BA</abbrev> (<xref ref-type="bibr" rid="B42">Möller et al. 2002</xref>). Follow-up studies showed that the protective effect was maintained for up to 10 years after therapy (<xref ref-type="bibr" rid="B44">Niggemann et al. 2006</xref>; <xref ref-type="bibr" rid="B29">Jacobsen et al. 2007</xref>).</p>
<p>Omalizumab is an anti-IgE antibody that received marketing authorization for use in asthma in 2005. Results from the pivotal clinical trial showed a statistically significant reduction in clinically relevant symptoms after 28 weeks of treatment in patients with severe persistent asthma (<xref ref-type="bibr" rid="B27">Humbert et al. 2005</xref>). Subsequent meta-analyses show that the use of omalizumab is also associated with a meaningful improvement in <abbrev xlink:title="allergic rhinitis" id="ABBRID0EHHAC">AR</abbrev> symptoms, overall quality of life, and a reduction in medication use for <abbrev xlink:title="allergic rhinitis" id="ABBRID0ELHAC">AR</abbrev> (<xref ref-type="bibr" rid="B61">Tsabouri et al. 2014</xref>; <xref ref-type="bibr" rid="B67">Yu et al. 2020</xref>).</p>
<p>Dupilumab is an anti-interleukin-4 (<abbrev xlink:title="anti-interleukin-4" id="ABBRID0EZHAC">IL-4</abbrev>) receptor alpha (<abbrev xlink:title="anti-interleukin-4 receptor alpha" id="ABBRID0E4HAC">IL-4Rα</abbrev>) antibody, which reduces Th2-type inflammation by decreasing the availability of the receptor for proinflammatory cytokines <abbrev xlink:title="anti-interleukin-4" id="ABBRID0EBIAC">IL-4</abbrev> and IL-13. In a randomized, double-blind, placebo-controlled study, dupilumab was shown to reduce corticosteroid use by 70% compared to a 42% reduction in the placebo group. Patients treated with dupilumab experienced 59% fewer asthma exacerbations compared to those receiving placebo (<xref ref-type="bibr" rid="B49">Rabe et al. 2018</xref>). Two post hoc analyses of the clinical data for dupilumab showed that patients with <abbrev xlink:title="allergic rhinitis" id="ABBRID0EJIAC">AR</abbrev> receiving dupilumab experienced a statistically significant reduction in symptoms (<xref ref-type="bibr" rid="B65">Wenzel et al. 2016</xref>; <xref ref-type="bibr" rid="B5">Busse et al. 2020</xref>).</p>

<p>In the case of asthma, it is known that dysregulated immune responses of different types can result in very similar symptoms. Endotypes of asthma include type 2-high asthma, which is characterized by high blood and sputum eosinophil counts (<xref ref-type="bibr" rid="B47">Ogulur et al. 2021</xref>). Type 2-low asthma is characterized by the absence of type 2 biomarkers and is often associated with the presence of sputum neutrophils (<xref ref-type="bibr" rid="B57">Sze et al. 2020</xref>). Efforts to understand the pathways behind type 2-low asthma are important, as this endotype represents 30–50% of patients with refractory asthma, and there are few specific therapy options available (<xref ref-type="bibr" rid="B25">Hinkset al. 2021</xref>).</p>
<p>Mutations affecting the IL17A gene have been found to be associated with both asthma and <abbrev xlink:title="allergic rhinitis" id="ABBRID0ECKAC">AR</abbrev> (<xref ref-type="bibr" rid="B64">Wang et al. 2012</xref>; <xref ref-type="bibr" rid="B14">Du et al. 2016</xref>; <xref ref-type="bibr" rid="B31">Lee and Song 2023</xref>). <p>IL17A is expressed in several immune cell types upon activation, including innate lymphoid cells (<abbrev xlink:title="innate lymphoid cells" id="ABBRID0EYMAC">ILC</abbrev>), natural killer (<abbrev xlink:title="natural killer" id="ABBRID0E3MAC">NK</abbrev>) cells, CD8⁺ T cells, γδ T cells, and, importantly, Th17 cells. It performs its intercellular signaling function as a homodimer or as a heterodimer with IL17F (<xref ref-type="bibr" rid="B36">Liu et al. 2013a</xref>; <xref ref-type="bibr" rid="B20">Goepfert et al. 2017</xref>). Receptors for the IL17A homo- or heterodimer are found in diverse tissues and organs; however, the best-characterized response to IL17A signaling occurs in epithelial cells, endothelial cells, and fibroblasts (<xref ref-type="bibr" rid="B19">Gaffen 2009</xref>; <xref ref-type="bibr" rid="B45">Nirula et al. 2016</xref>), where IL17A signaling precipitates expression of chemokines CXCL1 and CXCL8, cytokines IL1β, IL6, and TNF, and metalloproteases with direct antimicrobial functions (<xref ref-type="bibr" rid="B50">Rex et al. 2023</xref>). The chemokines and cytokines induced by IL17A attract and activate macrophages and neutrophils, which affect the removal of harmful pathogens in a protective immune response and contribute to the pathology of autoimmune or atopic diseases when dysregulated (<xref ref-type="bibr" rid="B40">Mills 2023</xref>).</p>
<p>Genome-wide association studies (<abbrev xlink:title="Genome-wide association studies" id="ABBRID0EEOAC">GWAS</abbrev>) have identified 128 mutations associated with asthma, including many that affect IL33, its receptor IL1RL1, or its downstream effectors (<xref ref-type="bibr" rid="B15">El-Husseini et al. 2020</xref>). IL33 is constitutively expressed in many tissues throughout the body, including the respiratory system (<xref ref-type="bibr" rid="B43">Moussion et al. 2008</xref>; <xref ref-type="bibr" rid="B17">Fagerberg et al. 2014</xref>). IL33 is released into the intercellular space in response to tissue or cell damage, where it induces an immune response, the specifics of which depend on the types of immune cells present and the cytokine milieu (<xref ref-type="bibr" rid="B18">Funakoshi-Tago et al. 2008</xref>; <xref ref-type="bibr" rid="B52">Salmond et al. 2012</xref>). IL33 is subject to several post-translational regulatory mechanisms, including nuclear localization during homeostasis (<xref ref-type="bibr" rid="B7">Carriere et al. 2007</xref>); activating proteolysis in the intercellular space (<xref ref-type="bibr" rid="B32">Lefrançais et al. 2014</xref>; <xref ref-type="bibr" rid="B54">Scott et al. 2018</xref>); deactivating proteolysis during apoptosis (<xref ref-type="bibr" rid="B35">Lingel et al. 2009</xref>; <xref ref-type="bibr" rid="B37">Liu et al. 2013b</xref>); deactivating oxidation in the intercellular space (<xref ref-type="bibr" rid="B35">Lingel et al. 2009</xref>; <xref ref-type="bibr" rid="B37">Liu et al. 2013b</xref>); and sequestration by circulating dummy receptors (<xref ref-type="bibr" rid="B24">Hayakawa et al. 2007</xref>). These mechanisms suggest IL33’s key role as a highly localized but generalized activating signal to the immune system in cases of tissue damage.</p>
<p>A series of experiments published in 2014 (<xref ref-type="bibr" rid="B41">Mizutani et al. 2014</xref>) suggests a mechanism for the interaction between IL33 and IL17A in <abbrev xlink:title="bronchial asthma" id="ABBRID0ETAAE">BA</abbrev>. Both anti-IL33 and anti-IL17A antibodies decrease neutrophilic inflammation in mice; however, the administration of anti-IL33 antibodies does not affect IL17A levels. Further, mice begin to express elevated levels of IL17A after repeated intrabronchial exposure to IL33. After sensitization by intrabronchial IL33 administration, the combined administration of IL33 and IL17A results in higher levels of neutrophilic inflammation than the administration of IL33 or IL17A alone. These results suggest an interaction in which repeated stresses expose the respiratory microenvironment to IL33, ultimately leading to activation of an IL17A-driven immune response that exacerbates inflammation triggered by future IL33-mediated responses to external stimuli. Subsequent publications appear to support this proposed interaction (<xref ref-type="bibr" rid="B63">Vocca et al. 2015</xref>; <xref ref-type="bibr" rid="B9">Cheng et al. 2022</xref>).</p>
<p>This hypothesis contends that, since <abbrev xlink:title="allergic rhinitis" id="ABBRID0E4BAE">AR</abbrev> and <abbrev xlink:title="bronchial asthma" id="ABBRID0EBCAE">BA</abbrev> are manifestations of the same inflammatory process, investigation of the expression levels of IL17A and IL33 in patients with <abbrev xlink:title="allergic rhinitis" id="ABBRID0EJCAE">AR</abbrev> and <abbrev xlink:title="bronchial asthma" id="ABBRID0ENCAE">BA</abbrev> may provide valuable insights into their interactions and help assess their value as biomarkers for different endotypes of the disease.</p>

      <title>Discussion</title>
<p>Our results suggest that patients with allergic rhinitis (<abbrev xlink:title="allergic rhinitis" id="ABBRID0EQ6AE">AR</abbrev>) and those with allergic rhinitis and bronchial asthma (<abbrev xlink:title="allergic rhinitis" id="ABBRID0EU6AE">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EY6AE">BA</abbrev>) form a homogeneous patient population characterized by increased IL-17A levels in both local (nasal lavage fluid; <abbrev xlink:title="nasal lavage fluid" id="ABBRID0E36AE">NLF</abbrev>) and systemic (serum) samples compared to healthy volunteers and decreased <abbrev xlink:title="interleukin-33" id="ABBRID0EBAAG">IL-33</abbrev> levels in both <abbrev xlink:title="nasal lavage fluid" id="ABBRID0EFAAG">NLF</abbrev> and serum compared to healthy volunteers. Notably, there were no statistically significant differences between the two patient populations. An additional difference between healthy volunteers and <abbrev xlink:title="allergic rhinitis" id="ABBRID0EJAAG">AR</abbrev> or <abbrev xlink:title="allergic rhinitis" id="ABBRID0ENAAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0ERAAG">BA</abbrev> patients was the increased <abbrev xlink:title="interleukin-33" id="ABBRID0EVAAG">IL-33</abbrev> levels in <abbrev xlink:title="nasal lavage fluid" id="ABBRID0EZAAG">NLF</abbrev> compared to serum, which was not observed in healthy volunteers. Limitations of this study include the small sample size and the fact that the observed numerical difference between <abbrev xlink:title="allergic rhinitis" id="ABBRID0E4AAG">AR</abbrev> serum IL-17A levels and healthy serum IL-17A levels was not statistically significant (p = 0.062). Additional research with a larger sample size is needed to improve the statistical power of these observations.</p>
<p>Gene expression data from three European birth cohorts show that rhinitis without additional atopic disease in patients aged 10–20 years has an identifiable and distinct pattern of gene expression clustered around IL17A and Toll-like receptor (<abbrev xlink:title="Toll-like receptor" id="ABBRID0EIBAG">TLR</abbrev>) overexpression, which does not overlap with the expression profile of patients with combined asthma, rhinitis, and/or atopic dermatitis (<xref ref-type="bibr" rid="B33">Lemonnier et al. 2020</xref>). Based on this information, our data may be interpreted as consistent with an <abbrev xlink:title="interleukin-17" id="ABBRID0EQBAG">IL-17</abbrev>–driven etiology for allergic rhinitis in the patients included in this study. Interestingly, the age of <abbrev xlink:title="allergic rhinitis" id="ABBRID0EUBAG">AR</abbrev> and <abbrev xlink:title="allergic rhinitis" id="ABBRID0EYBAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0E3BAG">BA</abbrev> patients in our study was higher than that reported by Lemonnier et al. (<abbrev xlink:title="allergic rhinitis" id="ABBRID0EACAG">AR</abbrev> patients: 29.6 ± 6.7; <abbrev xlink:title="allergic rhinitis" id="ABBRID0EECAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EICAG">BA</abbrev> patients: 36.8 ± 11.6). Our findings suggest that <abbrev xlink:title="interleukin-17" id="ABBRID0EMCAG">IL-17</abbrev>–driven <abbrev xlink:title="allergic rhinitis" id="ABBRID0EQCAG">AR</abbrev> may progress to <abbrev xlink:title="allergic rhinitis" id="ABBRID0EUCAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EYCAG">BA</abbrev> through as-yet-unknown mechanisms. Further research will be needed to determine the risk factors and biomarkers for the progression of IL17A-driven <abbrev xlink:title="allergic rhinitis" id="ABBRID0E5CAG">AR</abbrev> to <abbrev xlink:title="allergic rhinitis" id="ABBRID0ECDAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EGDAG">BA</abbrev>.</p>
<p>IL33 mRNA was elevated compared to healthy volunteers in both <abbrev xlink:title="allergic rhinitis" id="ABBRID0EODAG">AR</abbrev> and <abbrev xlink:title="allergic rhinitis" id="ABBRID0ESDAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EWDAG">BA</abbrev> patients. The decreased protein levels of <abbrev xlink:title="interleukin-33" id="ABBRID0E1DAG">IL-33</abbrev> in both patient groups compared to healthy volunteers therefore suggest the involvement of posttranscriptional regulatory mechanisms. <xref ref-type="bibr" rid="B59">Tang et al. (2018)</xref> reported that <abbrev xlink:title="interleukin-33" id="ABBRID0ECEAG">IL-33</abbrev> regulation by miR-200b and miR-200c is reduced in asthma patients and that the degree of this regulation is inversely correlated with asthma severity. As the patients in our study had mild or no asthma, it would be consistent to find that these microRNAs regulate <abbrev xlink:title="interleukin-33" id="ABBRID0EGEAG">IL-33</abbrev> expression.</p>
<p>Another major posttranscriptional regulatory mechanism affecting IL33 is the presence of soluble decoy receptors in the extracellular space. These decoy receptors, known as soluble suppressor of tumorigenicity 2 (<abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EOEAG">sST2</abbrev>), consist of the extracellular domain of IL1RL1, excluding the transmembrane and intracellular portions of the full protein. <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EUEAG">sST2</abbrev> recognizes and binds to <abbrev xlink:title="interleukin-33" id="ABBRID0EYEAG">IL-33</abbrev> at the same site as the active receptor IL1RL1, thereby blocking the formation of the <abbrev xlink:title="interleukin-33" id="ABBRID0E5EAG">IL-33</abbrev>/IL1RL1 receptor complex. It may, therefore, be the case that <abbrev xlink:title="allergic rhinitis" id="ABBRID0EEFAG">AR</abbrev> and <abbrev xlink:title="allergic rhinitis" id="ABBRID0EIFAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EMFAG">BA</abbrev> patients included in this study have increased <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EQFAG">sST2</abbrev> levels compared to healthy volunteers. <xref ref-type="bibr" rid="B68">Zhu et al. (2021)</xref> reported that <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EYFAG">sST2</abbrev> is elevated in patients with <abbrev xlink:title="allergic rhinitis" id="ABBRID0E3FAG">AR</abbrev> (19.8 ± 7.5 ng/mL). In addition, <xref ref-type="bibr" rid="B39">Mai et al. (2021)</xref> reported that the <abbrev xlink:title="interleukin-33" id="ABBRID0EEGAG">IL-33</abbrev><abbrev xlink:title="enzyme-linked immunosorbent assay" id="ABBRID0EIGAG">ELISA</abbrev> kit used in our experiments is tolerant to only 1 ng/mL of <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EMGAG">sST2</abbrev>. It is, therefore, likely that the <abbrev xlink:title="enzyme-linked immunosorbent assay" id="ABBRID0EQGAG">ELISA</abbrev> results were affected by the presence of <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EUGAG">sST2</abbrev> and that the <abbrev xlink:title="interleukin-33" id="ABBRID0EYGAG">IL-33</abbrev> protein levels in all samples were underestimated.</p>
<p>The apparent lack of posttranscriptional IL33 regulation in healthy volunteers may be due to differential <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EAHAG">sST2</abbrev> levels between the healthy and patient populations. Follow-up research should determine <abbrev xlink:title="suppressor of tumorigenicity 2" id="ABBRID0EEHAG">sST2</abbrev> levels to control for this confounding factor. If it is confirmed that IL33 levels are subject to greater regulation in the <abbrev xlink:title="allergic rhinitis" id="ABBRID0EKHAG">AR</abbrev> and <abbrev xlink:title="allergic rhinitis" id="ABBRID0EOHAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0ESHAG">BA</abbrev> patients enrolled in this study compared to healthy volunteers, this would raise the interesting possibility that these regulatory pathways are either instrumental in or a consequence of the etiology of IL17A-driven <abbrev xlink:title="allergic rhinitis" id="ABBRID0EYHAG">AR</abbrev>.</p>
<p>The existence of a population of patients who progress from <abbrev xlink:title="allergic rhinitis" id="ABBRID0E5HAG">AR</abbrev> alone to <abbrev xlink:title="allergic rhinitis" id="ABBRID0ECIAG">AR</abbrev> combined with <abbrev xlink:title="bronchial asthma" id="ABBRID0EGIAG">BA</abbrev> raises immediate interest in therapeutic options to prevent this progression. It may be the case that the pathological burden caused by <abbrev xlink:title="allergic rhinitis" id="ABBRID0EKIAG">AR</abbrev> is either causative for the progression to <abbrev xlink:title="bronchial asthma" id="ABBRID0EOIAG">BA</abbrev> or a necessary upstream factor in the process. Conventional therapies for <abbrev xlink:title="allergic rhinitis" id="ABBRID0ESIAG">AR</abbrev>, such as antihistamines or corticosteroids, would then likely prove effective in preventing progression to <abbrev xlink:title="bronchial asthma" id="ABBRID0EWIAG">BA</abbrev> in patients with <abbrev xlink:title="allergic rhinitis" id="ABBRID0E1IAG">AR</abbrev> who have elevated <abbrev xlink:title="interleukin-17" id="ABBRID0E5IAG">IL-17</abbrev> and <abbrev xlink:title="interleukin-33" id="ABBRID0ECJAG">IL-33</abbrev>.</p>
<p>Progression of <abbrev xlink:title="allergic rhinitis" id="ABBRID0EIJAG">AR</abbrev> patients to <abbrev xlink:title="allergic rhinitis" id="ABBRID0EMJAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EQJAG">BA</abbrev> in this population may also rely on a mechanism that does not respond to symptomatic treatment of <abbrev xlink:title="allergic rhinitis" id="ABBRID0EUJAG">AR</abbrev>. Marketed biologicals targeting <abbrev xlink:title="interleukin-17" id="ABBRID0EYJAG">IL-17</abbrev> pathway components – including secukinumab, ixekizumab, and brodalumab – may be effective in preventing progression to <abbrev xlink:title="allergic rhinitis" id="ABBRID0E3JAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EAKAG">BA</abbrev>. Precise epidemiological data will be necessary to assess whether the risk–benefit ratio of these medicines is favorable for an indication such as <abbrev xlink:title="allergic rhinitis" id="ABBRID0EEKAG">AR</abbrev>.</p>

This finding is consistent with recent studies and reveals a novel population of <abbrev xlink:title="allergic rhinitis" id="ABBRID0EGLAG">AR</abbrev>+<abbrev xlink:title="bronchial asthma" id="ABBRID0EKLAG">BA</abbrev> patients with elevated <abbrev xlink:title="interleukin-17" id="ABBRID0EOLAG">IL-17</abbrev> levels. The possibility of novel <abbrev xlink:title="interleukin-33" id="ABBRID0ESLAG">IL-33</abbrev> regulatory mechanisms in these patients warrants further research.</p>
